POSITION TITLE: CASE MANAGERDEPARTMENT (S): CASE MANAGEMENTREPORTS TO: DIRECTOR OF CASE MANAGEMENTEXEMPT/NON-EXEMPT: Non-exempt POSITION SUMMARY:Under the general supervision of the Director of Case Management, the Case Manager is responsible for assisting in the implementation of the Case Management Program to ensure ongoing assessment of patient care delivery according to established standards.
