Skilled Nursing | Rehabilitation | Long-Term Care
Just a 20-minute commute from Salem
Independence Health and Rehabilitation Center is seeking a compassionate RN to join our highly collaborative care team, supporting patients across skilled nursing, rehab, and long-term care as they transition from hospital to home and regain independence.
What You’ll Do:
Provide direct patient care (medications, treatments, monitoring)
Develop care plans in partnership with an interdisciplinary team
Assess patients and respond to changing needs
Work closely with nurses, therapists, and providers to achieve the best outcomes
Communicate with patients, families, and team members
Maintain accurate documentation
What You Need:
Active Oregon RN license
Strong teamwork and communication skills
Ability to thrive in a fast-paced environment
Experience in skilled nursing, rehab, or long-term care preferred
