Care Manager – Flexible Caseload & Competitive Pay:

Responsibilities:
Develop and implement care plans tailored to client needs
Coordinate services and resources to ensure optimal support
Maintain accurate and timely documentation
Communicate effectively with clients, families, and providers
Qualifications:
Experience in care management, social work, or a related field preferred
Strong organizational and communication skills
Ability to manage multiple cases efficiently
Must have a BA

Locations: Middletown, Bronx, Newburg, Monroe, Woodbury, Goshen, Beacon, Warwick, Queens, Rockland and Westchester and Surrounding areas.
