Job Summary:
Constant Care Health Services LLC is seeking a dedicated and compassionate Registered Nurse Supervisor to lead our evening shift team in The Bronx, New York. As a Registered Nurse Supervisor, you will play a vital role in ensuring the highest level of care for our patients while promoting a positive and supportive work environment for our home care professionals.
About the Role:
As a Registered Nurse Supervisor, you will be responsible for supervising a team of home care professionals, providing guidance and support to ensure the delivery of exceptional patient care. You will also be responsible for:
Coordinating patient care activities to ensure that all patients receive the highest level of care and support
Supervising and mentoring our home care professionals to promote professional growth and development
Maintaining effective communication with patients, families, and healthcare providers to ensure that all parties are informed of patient status and treatment plans
Identifying and addressing any concerns or issues related to patient care or staff performance
Responsibilities:
• Oversee patient care within the evening shift, ensuring that all patients receive timely and effective care
• Supervise and evaluate the performance of home care professionals, providing constructive feedback and coaching as needed
• Collaborate with other healthcare professionals to ensure that patients receive comprehensive and coordinated care
• Maintain accurate and timely records of patient care and staff performance
Requirements:
Current RN licensure in the State of New York
Minimum 2+ years of experience as a Registered Nurse, with experience in home care or long-term care preferred
Excellent communication and leadership skills, with the ability to work effectively with patients, families, and healthcare professionals
Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities
