What Makes This Position Special? Responsible for managing all aspects of resident satisfaction and care including identifying resources for health, social, and emotional needs and coordinating support services.
What You’ll Do:
- Responsible for admission, move-in, and orientation process for all new residents in independent and assisted living. Works with Sales/Marketing and Environmental Services to ensure smooth move-in process
- Provides support to new residents to ensure a smooth integration into community life and activities. Works with Resident Welcoming Committee, Wellness Team, Director of Creative Living, Chaplain, and others as needed to support resident life.
- Assists residents in problem solving and accessing needed services and implements programs and services to maintain or enhance resident independence including utilizing community resources.
- Participate in resident assessments and reassessments ensuring that the resident, family, and/or responsible party understands the process and results in a non-clinical fashion.
- Serve as the primary liaison between residents’ family members/responsible parties and team members to ensure that concerns are addressed. Maintain accurate records of interactions, issues, and resolutions.
- Collaborates with other team members to ensure quality of care and sets standards that create a sense of security for the residents.
- Encourages residents and family members/ responsible parties to take an active role in resident’s care including participation in development and maintenance of individual care plan.
- Coordinates resident transfers and discharges with resident, family members/responsible parties, and – team members including assuming possession of community property and completing necessary paperwork.
- Maintains and updates the Resident Directory to ensure accuracy and accessibility.
- Reviews and revises Resident Handbook as needed to reflect current policies and procedures
- Conducts investigations related to resident theft or loss incidents in a timely and professional manner
- Performs pet assessments and oversees the management of pet- related matters in accordance with community guidelines
- Oversees the resident survey process and coordinates follow-up as needed.
- Assigns and manages resident parking spaces according to community policies and availability
The Skills You’ll Need: Bachelor’s Degree in social work, gerontology, healthcare, or related field and 2+ years of experience working with the older adults, preferably in a long-term care setting or an equivalent combination of education and experience that provides the necessary knowledge, skills, and abilities.
