Summary: We’re hiring a full-time, 32-hour Licensed Social Worker (LSW) to serve as our Skilled Admissions Coordinator for short-term rehab and skilled nursing admissions for our 64-bed home. You will manage skilled payer-source referrals, coordinate clinical review and authorizations, and build strong relationships with hospitals and referral partners to drive census growth and ensure seamless transitions for residents and families.

Key Responsibilities:

  • Triage referrals and coordinate clinical reviews to support timely admission decisions.

  • Work with administration to verify payer source and obtain authorizations.

  • Collaborate with nursing leadership and the interdisciplinary team to assess clinical appropriateness and facility capacity.

  • Serve as primary liaison with hospitals, case managers, discharge planners, and community referral sources.

  • Communicate regularly with Social Services about bed availability, resident status, and potential long-term care conversions.

  • Support discharge planning and care transitions for skilled residents.

  • Maintain accurate referral, admission, census, and occupancy records and use electronic referral/EMR systems.

  • Develop and implement outreach strategies to increase skilled admissions and strengthen referral partnerships.

Required Qualifications:

  • Licensed Social Worker (LSW) — required.

  • Minimum 2 years’ experience in admissions, social services, case management, discharge planning, or related healthcare field preferred.

  • Solid knowledge of Medicare, Medicaid, managed care, and insurances

  • Experience with skilled nursing facility admissions, reimbursement systems, and long-term care regulations.

  • Excellent communication, relationship-building, and customer-service skills.

  • Strong organizational skills and ability to manage multiple priorities.

  • Proficiency with electronic referral platforms, EMRs, and healthcare software.

Preferred:

  • Existing hospital or community referral relationships.

  • Experience driving census growth through outreach and marketing activities.
