Job Summary:
Effingham Healthcare & Senior Living is seeking a compassionate and experienced Social Services Director to join our team. As a Social Services Director at Effingham Healthcare & Senior Living, you will be responsible for providing high-quality, person-centered care to our residents, while also overseeing the social services department and ensuring seamless admissions and transition processes.
About the Role:
As a Social Services Director at Effingham Healthcare & Senior Living, you will:
Provide quality compassionate medical social services to our residents, ensuring compliance with all regulations and standards
Coordinate admissions for new incoming residents, ensuring a smooth and welcoming experience for all new residents. Assists families with the move in process
Coordinate with healthcare professionals, family members, and other stakeholders to ensure resident-centered care
Provide emotional support and guidance to residents, family members, and staff
Oversee transitions to home, working with home health providers and service providers to ensure services needed are setup prior to discharge
Collaborate with other departments, such as nursing and activities, to ensure a comprehensive approach to care
