Job Title: Activity Assistant
Location: The Shores of Worthington, Skilled Nursing Facility

Job Type: Part-Time (Every Other weekend)

Hours: 9:30AM-2:30PM
Reports To: Activities Coordinator

Position Summary:

The Activity Assistant supports the planning and implementation of engaging and meaningful activity programs for residents in a skilled nursing facility. This role contributes to residents’ physical, mental, and emotional well-being by encouraging participation in a variety of recreational, social, and therapeutic activities.

Key Responsibilities:

  • Assist in developing and executing daily, weekly, and monthly activity calendars that meet the diverse needs and interests of residents.

  • Lead group and one-on-one activities such as games, arts and crafts, music, exercise sessions, outings, and social events.

  • Encourage resident participation and promote a positive and inclusive environment.

  • Transport or escort residents to and from activities as needed, ensuring safety at all times.

  • Help decorate and set up for events, holidays, and themed programs.

  • Maintain attendance records, activity documentation, and progress notes as required by facility policy and regulations.

  • Collaborate with nursing staff, volunteers, and families to support individual care plans and preferences.

  • Assist with developing individualized activity plans based on residents’ cognitive and physical abilities.

  • Monitor residents during activities and report any changes in mood, behavior, or condition to appropriate staff.

Qualifications:

  • High school diploma or GED required.

  • Previous experience in an activities or recreation role in a healthcare setting preferred.

  • Compassionate and patient demeanor with excellent interpersonal skills.

  • Creative, energetic, and organized with a positive attitude.

  • Ability to work independently and as part of a team.

  • Ability to push wheelchairs and assist with resident mobility when necessary.

  • Must be able to work flexible hours, including evenings, weekends, and holidays.

  • CPR/First Aid certification a plus.

  • Must pass background check and meet all facility/state health requirements.

Physical Requirements:

  • Frequent standing, walking, bending, lifting (up to 25 lbs).

  • Ability to assist with physical tasks during activities and resident care.
