Department: Day Habilitation ServicesReports To: Day Habilitation Program ManagerExempt/Non-Exempt: ExemptSalary: $48,000 - $50,000 YEARLYLocation: The League for People with Disabilities – Baltimore, MDSummary:The DDA Case Manager plays a vital role in supporting adults with developmental and intellectual disabilities to lead independent, fulfilling, and community-connected lives. This position ensures each participant's Person-Centered Plan (PCP) is implemented with integrity and aligned with DDA standards and The League's mission to help individuals "achieve independence and self-determination."The Case Manager serves as the primary advocate and liaison between participants, their families, interdisciplinary teams, and community partners-driving quality outcomes through coordination, communication, and compassion.Essential Duties and Responsibilities:Coordinate and Monitor Services:Develop, implement, and monitor individualized Person-Centered Plans (PCPs) in collaboration with participants, families, and care teams.Ensure services align with DDA regulations, goals, and participant preferences.Advocate and Support:Serve as a key advocate for participants' rights, choices, and inclusion within Day Habilitation and the broader community.Facilitate ongoing communication between families, residential providers, and community partners to ensure consistent care.Documentation and Compliance:Maintain accurate, timely, and compliant records in accordance with DDA, Medicaid, and agency standards.Prepare reports, progress notes, and assessments that demonstrate measurable outcomes and service impact.Team Collaboration:Partner with Day Hab staff, clinicians, and support professionals to ensure holistic and person-centered service delivery.Participate in interdisciplinary team meetings and case reviews to adjust plans as participant needs evolve.Quality Assurance and Improvement:Identify and address barriers to participant success, contributing to program improvements and best practice sharing.Support DDA audits and reviews by ensuring documentation accuracy and readiness.Community Engagement:Connect participants with community resources and opportunities for employment, recreation, and volunteerism.Promote The League's inclusive culture by modeling respect, empathy, and empowerment in every interaction.Qualifications:Education: Bachelor's degree in Social Work, Psychology, Human Services, or a related field required.Experience: Minimum 2 years of experience providing case management or support coordination for individuals with developmental or intellectual disabilities preferred.Knowledge:Understanding of DDA regulations, person-centered planning, and community integration principles.Familiarity with Medicaid waiver services and electronic documentation systems.Skills:Strong organizational, communication, and advocacy skills.Ability to build trust and rapport with diverse individuals and families.Proficiency in Microsoft Office Suite and data tracking systems.Other Requirements:Valid driver's license, reliable transportation, and ability to pass background checks.Core Competencies:Compassionate AdvocacyPerson-Centered ThinkingCollaboration and TeamworkAccountability and Attention to DetailCultural CompetenceAdaptability and Problem-Solving
