Mental Health Case Manager
Job Description
The Position:St. Patrick Center, a ministry of Catholic Charities of the Archdiocese of St. Louis, is hiring a Mental Health Case Manager to join our Housing, Employment and Recovery Opportunities (HERO) program and support individuals on their recovery journey. This role provides mental health assessments, counseling, and case management services to adults with serious mental illness and co‑occurring substance use challenges. Working as part of a multidisciplinary team, the case manager helps develop treatment plans, coordinate services, and connect clients with vital community resources.
