Summary:

Twin Oaks Rehab and Nursing is seeking a talented and compassionate Staff LPN to join our dynamic team. As a Staff LPN, you will play a vital role in providing exceptional care to our residents. Responsibilities:

• Provide direct patient care, including administering medications, wound care, and other medical treatments as needed

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Monitor and report changes in patient condition to the nurse supervisor and other healthcare professionals

• Participate in quality improvement initiatives to ensure high-quality patient care

• Maintain accurate and up-to-date patient records

Requirements:

• Ability to work varied shifts, including days, evenings, and nights

• Excellent communication and interpersonal skills

• Strong attention to detail and ability to prioritize tasks effectively

• New graduates and experienced LPNs encouraged to apply
