Responsibilities:

• Serve as a liaison between patients, families, and healthcare providers to ensure seamless communication and care coordination

• Develop and maintain strong relationships with patients, families, and healthcare partners to ensure high-quality care and satisfaction

• Assist with patient intake, registration, and scheduling to ensure efficient and timely care delivery

• Monitor and report patient flow and throughput to optimize care delivery and reduce wait times

• Collaborate with care teams to identify and address patient needs, including medical, emotional, and spiritual support

• Provide education and support to patients and families on treatment options, medication management, and disease prevention

• Participate in quality improvement initiatives to continually improve patient outcomes and satisfaction

Requirements:

• High school diploma or equivalent required; degree in a related field (e.g., healthcare, social work) preferred

• 0 years of experience in patient care or a related field

• Excellent communication, interpersonal, and problem-solving skills

• Ability to work effectively in a fast-paced environment with multiple priorities and deadlines
