We are seeking a compassionate and motivated Licensed Master Social Worker (LMSW) to join our team. This is an in-office position providing telehealth services to patients.
Responsibilities
Conduct patient assessments and intakes
Provide individual counseling and behavioral health support
Develop and follow patient care plans
Coordinate care with healthcare providers
Maintain accurate, HIPAA-compliant documentation
Provide case management and connect patients with appropriate resources
Qualifications
Active New York State LMSW license
Strong communication and interpersonal skills
Experience with behavioral health, counseling, or case management preferred
Comfortable providing services via telehealth
Organized and able to manage multiple patients
