MDS Coordinator Major Tasks and Responsibilities:

  • Coordinate and complete MDS assessments in accordance with CMS guidelines and facility policies.

  • Ensure timely and accurate completion of all MDS, Care Area Assessments (CAAs), and care plans.

  • Collaborate with interdisciplinary team members to develop and update individualized resident care plans.

  • Monitor clinical documentation to support MDS coding accuracy and reimbursement.

  • Review resident records and conduct assessments to determine care needs and services.

  • Track assessment schedules and maintain compliance with required submission deadlines.

  • Participate in Medicare, Medicaid, and insurance reviews as needed.

  • Educate nursing staff regarding documentation requirements and regulatory compliance.

  • Monitor quality measures and identify opportunities for clinical improvement.

  • Assist with survey preparedness and regulatory audits.

  • Collaborate with facility leadership to ensure optimal reimbursement and resident outcomes.

  • Maintain confidentiality of resident information in accordance with HIPAA regulations.

MDS Coordinator Required Education / Experience:

  • Current Registered Nurse (RN) license in good standing.

  • Minimum of 1-2 years of MDS experience in a skilled nursing or long-term care setting.

  • Strong knowledge of MDS 3.0, RAI process, and Medicare/Medicaid regulations.

  • Excellent assessment, organizational, and communication skills.

  • Ability to work independently and manage multiple deadlines.
