Position Overview

Casper Mountain Rehabilitation & Care Center is seeking a compassionate, dependable, and clinically skilled Registered Nurse (RN) to join our nursing team.

As an RN, you will provide comprehensive nursing care while serving as a clinical leader for residents and nursing team members. You will assess resident conditions, administer medications and treatments, coordinate care, supervise nursing personnel, and respond to changes in condition using sound clinical judgment.

Working collaboratively with physicians, CNAs, therapy, pharmacy, dietary, social services, and other members of the interdisciplinary team, you will help ensure residents receive safe, individualized, and high-quality care.

Key Responsibilities

Clinical Care & Resident Assessment

  • Provide comprehensive nursing care in accordance with individualized care plans, physician orders, clinical standards, and facility policies

  • Complete nursing assessments and continually monitor residents for changes in physical, cognitive, behavioral, and functional condition

  • Identify significant changes in condition and initiate appropriate nursing interventions and provider notifications

  • Administer medications, treatments, IV therapy, and other therapeutic interventions within applicable scope of practice

  • Evaluate resident responses to medications, treatments, and nursing interventions and modify the nursing approach as appropriate

  • Review laboratory results, diagnostic findings, physician orders, and other clinical information and ensure appropriate follow-up

  • Participate in admissions, readmissions, transfers, discharges, and transitions of care

  • Respond to urgent and emergency situations using appropriate nursing judgment and established clinical protocols

  • Promote resident comfort, dignity, independence, preferences, and quality of life throughout care delivery

Clinical Leadership & Staff Supervision

  • Provide clinical leadership and direction to LPNs, CNAs, and nursing support personnel during assigned shifts

  • Appropriately delegate nursing responsibilities based on resident needs, staff competency, and applicable scope-of-practice requirements

  • Monitor the quality and completion of delegated resident-care responsibilities

  • Provide coaching, education, and real-time clinical guidance to nursing team members

  • Promote accountability, teamwork, professionalism, and effective communication throughout the nursing department

  • Assist nursing leadership with identifying staffing, performance, education, and resident-care concerns

  • Support the orientation, training, and development of newly hired nursing personnel

  • Serve as a clinical resource and professional role model for members of the nursing team

Care Coordination & Communication

  • Collaborate with physicians, advanced practice providers, pharmacy, therapy, dietary, social services, and other interdisciplinary professionals

  • Coordinate resident care to ensure physician orders, treatments, services, and clinical recommendations are implemented appropriately

  • Participate in interdisciplinary care planning and resident-care conferences as assigned

  • Communicate significant changes in condition promptly to physicians, responsible parties, and appropriate clinical leadership

  • Provide residents and families with appropriate education regarding diagnoses, medications, treatments, and care plans

  • Maintain professional, compassionate, and transparent communication with residents and families

  • Ensure effective shift-to-shift communication to promote continuity of care and resident safety

Documentation & Regulatory Compliance

  • Maintain accurate, complete, and timely nursing documentation within the electronic medical record (EMR)

  • Ensure assessments, medication administration, treatments, physician notifications, changes in condition, and nursing interventions are appropriately documented

  • Maintain compliance with applicable CMS requirements, Wyoming regulations, facility policies, and professional nursing standards

  • Protect resident rights, confidentiality, privacy, dignity, and personal preferences

  • Report accidents, incidents, allegations, injuries, significant changes, and other reportable events in accordance with facility requirements

  • Support ongoing survey readiness by maintaining consistent clinical practices and accurate documentation

Quality, Infection Prevention & Resident Safety

  • Follow and reinforce infection prevention and control practices, including hand hygiene, PPE, and transmission-based precautions

  • Participate in Quality Assurance and Performance Improvement (QAPI) activities and clinical performance initiatives

  • Support facility programs addressing falls, pressure injuries, infection prevention, medication safety, rehospitalization reduction, and other clinical quality measures

  • Identify potential safety concerns and implement appropriate interventions

  • Participate in audits, investigations, clinical reviews, and corrective action initiatives as assigned

  • Promote a culture of resident safety, clinical accountability, regulatory compliance, and continuous improvement

Qualifications
  • Graduate of an accredited professional nursing program

  • Current, active Registered Nurse (RN) license in good standing in the State of Wyoming

  • Current CPR or Basic Life Support (BLS) certification

  • Previous experience in skilled nursing, long-term care, rehabilitation, post-acute care, or a related healthcare environment preferred

  • Strong nursing assessment, clinical judgment, critical-thinking, and decision-making abilities

  • Ability to recognize, evaluate, and respond appropriately to changes in resident condition

  • Demonstrated ability to supervise, delegate, coach, and support nursing personnel

  • Excellent written and verbal communication skills

  • Strong organizational and time-management capabilities

  • Ability to prioritize multiple clinical responsibilities in a fast-paced healthcare environment
