Job Title: Licensed Social Worker (LSW) – Skilled Nursing Facility (SNF)

Location: Crockett, Texas
Employment Type: Part-Time
Reports To: Administrator

Position Summary:

Winfield Rehab is seeking a compassionate and experienced Licensed Social Worker (LSW) to join our team in our skilled nursing facility in Crockett, Texas. The ideal candidate will provide psychosocial support and discharge planning services to residents and their families, ensuring quality of life and compliance with federal, state, and facility regulations.

Key Responsibilities:

  • Conduct psychosocial assessments and develop individualized care plans for new and existing residents

  • Provide crisis intervention, supportive counseling, and advocacy for residents and families

  • Coordinate discharge planning, including referrals to home health, hospice, or other post-acute care settings

  • Facilitate family meetings, care conferences, and interdisciplinary team collaboration

  • Assist residents with advance directives, financial resources, guardianship issues, and Medicaid applications

  • Maintain accurate and timely documentation in compliance with regulatory standards

  • Educate staff on residents’ psychosocial needs and behavioral interventions

  • Participate in quality assurance initiatives, surveys, and audits as required

Qualifications:

  • Current LBSW, LMSW, or LCSW license in good standing with the Texas State Board of Social Work Examiners

  • Minimum 1–2 years of experience in a skilled nursing facility (SNF) or long-term care setting

  • Strong knowledge of state and federal regulations related to SNF (e.g., CMS, OBRA)

  • Excellent communication, organizational, and interpersonal skills

  • Proficiency in electronic medical records (EMR) and case documentation

  • Ability to work independently and as part of an interdisciplinary team

Preferred Qualifications:

  • Experience with Medicaid/MCO processes

  • Familiarity with care transitions and community resources in Texas
