Job Summary
Altra Home Health is seeking a compassionate and skilled Nurse to join our team. In this rewarding role, you’ll provide personalized care to pediatric patients in their homes — many of whom have complex medical conditions, including gastrostomy tubes (G-tubes), cerebral palsy, chronic diseases, and occasionally require tracheostomy or ventilator support. You’ll play a vital role in helping them live safely and comfortably in their home, where they thrive the most.
Responsibilities
Perform thorough head-to-toe assessments and follow individualized plans of care
Provide care which may include G-tube feedings, medication administration, wound care, trach care and ventilator support.
Care for patients with professionalism and empathy.
Educate patients and caregivers on disease management, emergency procedures, and therapeutic techniques
Collaborate closely with physicians, therapists, and other team members to optimize care
Document all care activities accurately and promptly in compliance with agency and state requirements
Maintain rigorous safety, infection control, and privacy standards
Shift Information
Shift durations range from 4 to 12 hours, depending on patient needs and nurse availability
Flexible scheduling options available for daytime, nighttime, evening, and weekend shifts
Opportunities for consistent long-term cases or variable schedules
Qualifications
Active Nursing license in Indiana or an active compact Nursing license
At least [1–2] years of clinical nursing experience (home health and pediatric experience strongly preferred)
Experience with G-tube, trach, or vent care preferred (but training will be offered)
Current CPR certification
Strong critical thinking and independent decision-making abilities
Excellent interpersonal and communication skills
Reliable transportation and valid driver’s license
