Responsibilities:
Provide high-quality care and support to clients in their homes, ensuring their physical, emotional, and social needs are met
Develop and implement individualized care plans, in collaboration with clients and their families
Assist with daily living activities, including bathing, dressing, grooming, and feeding
Manage medication schedules and provide medication reminders
Maintain accurate records and documentation of client care
Collaborate with other healthcare professionals to ensure seamless care transitions
Provide emotional support and companionship to clients and their families
