Responsibilities:
Provide personalized care and support to residents, promoting independence and dignity
Assist with daily living activities, such as bathing, dressing, and grooming
Take vital signs, including temperature, pulse, and blood pressure
Assist with toileting, mobility, and incontinence care
Assist with medication administration, as needed
Collaborate with other care team members to develop and implement individualized care plans
Maintain accurate and timely records of resident care and services
Participate in care conferences and staff meetings to discuss resident care and updates
