We are currently seeking a detail-oriented and experienced MDS Coordinator to join our team!
Responsibilities:
- Complete and oversee accurate and timely MDS assessments in accordance with federal and state regulations
- Coordinate care plans and ensure compliance with Medicare/Medicaid guidelines
- Collaborate with interdisciplinary team to optimize resident care and reimbursement
- Monitor documentation and ensure accuracy for audits and reviews
- Stay up to date with regulatory changes and MDS guidelines
Qualifications:
- Active RN or LPN license
- Previous MDS experience in LTC/SNF preferred
- Strong knowledge of RAI process and care planning
- Excellent attention to detail and organizational skills
- Ability to work collaboratively with clinical staff
