Provide direct patient care to residents, including administering medications, performing wound care, and conducting vital signs
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Monitor and report changes in resident condition to ensure timely and effective interventions
Collaborate with other healthcare professionals to ensure seamless transitions and coordination of care
Participate in quality improvement initiatives to enhance the overall quality of life for our residents
