Responsibilities:

• Provide personal care and assistance with daily living activities, such as bathing, dressing, and grooming

• Assist with medication management and meal preparation, as needed

• Support clients with mobility and mobility-related activities, such as transfers and ambulation

• Encourage and promote clients' independence and autonomy

• Develop and implement personalized care plans in collaboration with clients, families, and healthcare teams

• Maintain accurate and detailed records of client care and progress

• Communicate effectively with clients, families, and healthcare teams to ensure seamless delivery of care
