SeaView Community Services is seeking a compassionate, dependable, and highly organized Case Manager to join our Recovery Center team. This position supports adults experiencing substance use and co-occurring behavioral health needs as they work toward recovery, stability, and greater independence.


The Case Manager works closely with clients, clinicians, peer support professionals, community providers, and referral partners to identify needs, reduce barriers, coordinate services, and help clients achieve their individualized treatment goals. The ideal candidate believes in each person’s capacity for change and can balance empathy with accountability, professional boundaries, and consistent follow-through.


Primary Responsibilities
  • Develop supportive, professional relationships with clients grounded in dignity, respect, and trauma-informed care.
  • Complete case management assessments and help clients identify strengths, needs, goals, and barriers to recovery.
  • Coordinate access to housing, healthcare, employment, education, transportation, benefits, legal services, and other community resources.
  • Collaborate with the clinical team to support individualized treatment and discharge plans.
  • Help clients develop practical skills related to daily living, problem-solving, self-advocacy, and community integration.
  • Monitor client progress and communicate changes, concerns, and barriers to the treatment team.
  • Provide outreach and follow-up to promote engagement and continuity of care.
  • Maintain timely, accurate, and compliant documentation in the electronic health record.
  • Participate in team meetings, case consultations, training, and quality-improvement activities.
  • Respond appropriately to urgent client needs and follow established safety and crisis-response procedures.
  • Transport or accompany clients to appointments and community resources when required.
  • Maintain client confidentiality and comply with organizational policies, ethical standards, and applicable regulations.
Requirements:
  • Bachelor’s degree in social work, psychology, human services, behavioral health, or a related field required.
  • Relevant experience in behavioral health, substance use treatment, case management, social services, or community-based care may be considered.
  • Knowledge of recovery-oriented, person-centered, and trauma-informed practices.
  • Strong interpersonal, organizational, communication, and problem-solving skills.
  • Ability to work effectively with individuals from diverse backgrounds and with complex needs.
  • Ability to maintain professional boundaries while building trusting relationships.
  • Ability to work both independently and collaboratively as part of an interdisciplinary team.
  • Proficiency with electronic documentation and basic computer applications.
  • Valid driver’s license and ability to meet SeaView’s driving requirements.
  • Ability to complete required background checks and meet all conditions of employment.
