Job Summary:

A well-established and highly regarded post-acute care organization is seeking an experienced MDS Coordinator (RN) to join its clinical leadership team. This is a confidential search for a key role responsible for ensuring the accuracy, compliance, and integrity of resident assessments and care planning.

This position offers the opportunity to work within a supportive environment that prioritizes quality outcomes, regulatory excellence, and strong interdisciplinary collaboration.

Key Responsibilities:

  • Complete accurate MDS assessments and comprehensive care plans

  • Initiate and oversee care planning processes and related activities

  • Develop and distribute monthly care plan schedules in a timely manner

  • Maintain and update all clinical assessments in accordance with regulations

  • Conduct ongoing audits of clinical documentation for accuracy and compliance

  • Partner with nursing leadership to address and resolve documentation issues

  • Ensure strict confidentiality of all resident and organizational information

Requirements:

  • Prior MDS 3.0 experience required

  • Active New Jersey RN License

  • Previous Long-Term Care experience required

  • Strong organizational and time management skills

  • Proficiency with clinical and documentation systems

  • Excellent communication and collaboration abilities

  • Team-oriented with a professional demeanor
