Job Description:
We are seeking a compassionate and dedicated Home Health Caregiver to join our team of healthcare professionals. As a Home Health Caregiver with Modani Care, you will have the opportunity to work in a supportive and collaborative environment, where your skills and expertise are valued and respected.
Responsibilities:
Provide high-quality care and support to clients in their homes, focusing on their physical, emotional, and social needs
Assist with daily living activities such as bathing, dressing, grooming, and toileting
Assist with mobility and transfers, including walkers, canes, and wheelchairs
Provide medication reminders and assistance with medication management
Collaborate with our interdisciplinary care team to develop and implement individualized care plans
Promote independence, dignity, and well-being of clients, while encouraging social interaction and engagement
