Job Description
Levering Regional Health Care Center is seeking a compassionate and dependable Counselor to provide emotional and psychosocial support to residents in a long-term care setting. The Counselor works closely with residents, families, and the interdisciplinary care team to support residents' emotional well-being, adjustment, and quality of life.
ResponsibilitiesProvide individual counseling and emotional support to residents.
Assess residents' psychosocial and emotional needs.
Develop and implement appropriate counseling interventions.
Participate in resident care planning and interdisciplinary team meetings.
Document counseling sessions, assessments, interventions, and resident progress.
Assist residents with adjustment to changes in health, independence, and living environment.
Communicate with residents, families, caregivers, and healthcare professionals regarding resident needs.
Identify behavioral, emotional, or psychosocial concerns and coordinate appropriate follow-up.
Maintain accurate and confidential resident records.
Follow facility policies, procedures, and applicable regulatory requirements.
Collaborate with nursing, social services, activities, and other departments to promote resident well-being.
Bachelor's degree in counseling, social work, psychology, or a related field; master's degree preferred.
Previous experience providing counseling or psychosocial services preferred.
Experience working with older adults or in a healthcare/long-term care setting preferred.
Strong communication, interpersonal, and documentation skills.
Ability to work effectively as part of an interdisciplinary team.
Ability to maintain confidentiality and handle sensitive information professionally.
