Hospice Nurse PractitionerFull-TimeParker Jewish Institute Comprehensive Community Hospice is seeking dedicated and compassionate full-time Nurse Practitioner to join our team. We want to speak with naturally caring, empathetic NPs who are focused on providing the best quality of end-of- life care possible for every patient. The Hospice Nurse Practitioner will be able to provide nursing care to the terminally ill Hospice patient, provide assistance and understanding to the family in the home care situation and in time of bereavement, and work as a member of the Hospice team in providing Hospice care.Community and Inpatient Hospice at Parker Jewish Institute provides comfort, relief, and peace of mind to individuals with advanced or life-limiting illness and their families. We are an intimate and unique not-for-profit program that provides personalized care to ensure the comfort, enhance the quality of life, and preserve the dignity of our patients. Your role will be to ensure these needs are met by conducting encounters with our patients to determine their initial and continued eligibility for hospice services.
