Luna Rehabilitation & Care Center is seeking a compassionate, dependable, and clinically skilled Registered Nurse (RN) to join our skilled nursing team.
We are offering a $2,500 Sign-On Bonus for eligible Full-Time Nurses.
As an RN, you will provide direct nursing care while serving as a clinical leader for residents and nursing team members. You will be responsible for resident assessments, medication and treatment administration, changes in condition, clinical documentation, staff supervision, and coordination of care.
Our RNs play a critical role in ensuring residents receive safe, timely, individualized, and high-quality nursing care. You will work closely with physicians, CNAs, nursing leadership, therapy, pharmacy, dietary, social services, and other members of the interdisciplinary team to promote positive clinical outcomes and quality of life.
Key ResponsibilitiesClinical Care & Resident Oversight
Provide comprehensive nursing care in accordance with individualized care plans, physician orders, professional nursing standards, and facility policies
Perform ongoing nursing assessments and identify changes in residents' physical, cognitive, behavioral, and psychosocial conditions
Initiate appropriate nursing interventions and promptly communicate significant changes in condition to physicians, responsible parties, and appropriate interdisciplinary team members
Safely prepare and administer medications, treatments, and therapeutic procedures within the RN scope of practice
Monitor resident responses to medications, treatments, and clinical interventions
Respond promptly to acute changes in condition, urgent needs, and emergency situations using sound clinical judgment
Assist with admissions, readmissions, transfers, and discharges as assigned
Ensure physician orders, laboratory results, diagnostic findings, and treatment recommendations receive appropriate follow-up
Promote resident dignity, independence, preferences, safety, and person-centered care
Nursing Leadership & Staff Supervision
Supervise, delegate, and provide clinical direction to CNAs and other nursing personnel
Make appropriate assignments based on resident needs, staff competency, and scope of practice
Monitor the quality and timeliness of care provided by nursing support staff
Provide real-time coaching, clinical guidance, education, and support to nursing team members
Promote accountability, teamwork, professionalism, and respectful communication throughout the nursing department
Identify and communicate resident care, staffing, performance, and safety concerns to nursing leadership
Support orientation and development of newly hired nursing team members as assigned
Documentation & Care Coordination
Maintain accurate, complete, and timely documentation within the electronic medical record (EMR)
Ensure assessments, medications, treatments, changes in condition, physician notifications, and resident responses are appropriately documented
Collaborate with physicians and interdisciplinary team members to coordinate resident care
Participate in resident care planning and interdisciplinary meetings as assigned
Provide appropriate education to residents and families regarding medications, treatments, care plans, and health-related concerns
Maintain effective communication during shift-to-shift reporting to promote continuity of care
Quality, Compliance & Resident Safety
Maintain compliance with applicable CMS requirements, New Mexico regulations, facility policies, and professional nursing standards
Follow infection prevention practices, including hand hygiene, appropriate PPE utilization, isolation precautions, and transmission-based precautions
Participate in Quality Assurance and Performance Improvement (QAPI) initiatives
Support facility efforts related to fall prevention, infection prevention, pressure injury prevention, medication safety, and reduction of avoidable hospitalizations
Immediately report incidents, accidents, allegations, resident safety concerns, or other reportable events according to facility policy and applicable requirements
Protect resident rights, privacy, dignity, confidentiality, and personal preferences
Support ongoing survey readiness and adherence to facility clinical standards
Graduate of an accredited nursing program
Current, active Registered Nurse (RN) license in good standing in the State of New Mexico
Current CPR or Basic Life Support (BLS) certification
Skilled nursing, long-term care, rehabilitation, or post-acute nursing experience preferred
Strong clinical assessment, critical-thinking, and decision-making skills
Ability to recognize and appropriately respond to changes in resident condition
Demonstrated ability to supervise, delegate, coach, and support nursing personnel
Excellent communication and interpersonal skills
Strong organizational and time-management abilities
Ability to effectively prioritize multiple clinical responsibilities in a fast-paced healthcare environment
Ability to collaborate professionally with residents, families, physicians, and interdisciplinary team members
