The Veteran and Family Wellness Program (VFWP) Resource Navigator will provide services that include connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care, specialized case management, care coordination, health education, interdisciplinary collaboration, coordination, and consultation, and administrative duties. Incumbent will work closely with the Veteran’s or Homeless individual’s primary care provider and members of the Individual’s assigned interdisciplinary treatment team. The Veteran and Family Wellness Program Resource Navigator as the representative of the American GI Forum NVOP will act as a liaison between the VA medical clinic and other community medical services. The Veteran and Family Wellness Program Resource Navigator will work with a population of Veteran/Homeless Individuals with complex needs who require assistance accessing health care services or adhering to health care plans and be willing to help them with obtaining health care insurance.
Job Requirements:
- Must have a Bachelor’s Degree with at least 2 years’ work experience in a health care or social services area of practice or equivalent experience
- Must have documented experience working with veterans, be a veteran themselves, or veteran spouse are preferred.
- Must keep timely documentation in an electronic medical records (EMR) system
- Must be willing to attend periodic training as required by the position.
- Must be available to begin immediately and carry professional liability insurance.
- Must be willing to travel within the local city in a privately owned vehicle.
- Must have a valid driver’s license and vehicle general liability insurance is required.
- Must be able to pass a drug test and background check.
- Clearance must be obtained prior to hiring.

Good understanding of the multitude of problems faced by veterans/clients

EDUCATION and/or EXPERIENCE
Must have a Bachelor’s Degree with at least 2 years work experience in healthcare or social area of practice.

- Must have access to reliable transportation
- Must have a current Texas driver’s license at all times
- Must have applicable automobile liability insurance at all times
- Must pass drug testing and background check
- All personnel of the organization must be bondable
- This position requires local travel on company time, and therefore must pass a driving record clearance by insurance carrier for this position.

WORK ENVIRONMENT
While performing the duties of this position, the employee will conduct office work, meet with clientele and team members, and at times travel offsite to conduct business. Travel by automobile will be necessary, and employee may be exposed to varying weather conditions. Employee will also be required to work in varied office and residential environment based on assigned caseload. Employee will be able to handle and de-escalate situations that may arise while at work. Employee will be required to recruit clients in traditional work environments such as job fairs and while networking at community events, as well as recruiting homeless clients where they may reside such as shelter homes, streets, and encampments.
