MDS Coordinator

Job Summary:

We are seeking a dedicated and compassionate Registered Nurse MDS Coordinator to join our team at Fulton Manor Care Center, a leading provider of Short Term Therapy and Long Term Care in Fulton, MO. As an integral member of our care team, the MDS Coordinator will play a vital role in coordinating the care of our residents, ensuring that their physical, emotional, and social needs are met.

About the Role:

  • Coordinate the care planning process for residents, collaborating with healthcare professionals, families, and other stakeholders to develop and implement individualized care plans

  • Assist with assessing and documenting resident needs, preferences, and goals

  • Facilitate effective communication among healthcare professionals, families, and other care providers to ensure seamless care coordination

  • Identify and address any gaps or inconsistencies in care, working to resolve issues promptly and efficiently

  • Maintain accurate and up-to-date records of resident care, including MDS assessments, care plans, and progress notes

Responsibilities:

  • Collaborate with the interdisciplinary team to develop and implement resident care plans

  • Conduct MDS assessments and develop care plans in accordance with current regulations and standards

  • Assist with implementing and evaluating resident care programs and services

  • Participate in quality improvement initiatives to improve care outcomes and resident satisfaction

  • Maintain confidentiality and adhere to HIPAA guidelines in handling resident information

  • Perform other duties as assigned by the Director of Nursing or other administrative staff

Requirements:

  • RN licensure with current registration in the State of Missouri

  • MDS Coordinator experience or related experience in healthcare setting

  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations and caregivers

  • Proficiency with electronic medical records (EMRs) and other healthcare software

  • Ability to work in a fast-paced environment, prioritizing multiple tasks and competing demands

  • Strong organizational and time management skills, with attention to detail and accuracy
