About the Role
As a RN Registered Nurse at Parkhouse Nursing and Rehab Center, you will be responsible for:
Providing direct patient care to our residents, including assessing, planning, implementing, and evaluating individualized care plans
Collaborating with our interdisciplinary team to ensure seamless communication and coordination of care
Developing and implementing effective care protocols to promote optimal patient outcomes
Utilizing our electronic medical record system to document patient information and track progress
Participating in ongoing education and training to stay current with best practices and industry developments
Responsibilities
Provide skilled nursing care to our residents, including wound care, ostomy care, and medication management
Conduct initial and ongoing assessments to identify care needs and develop individualized care plans
Implement and monitor care plans, making adjustments as necessary to ensure optimal outcomes
Communicate effectively with residents, families, and interdisciplinary team members to ensure coordinated care
Participate in quality improvement initiatives to drive better patient outcomes and resident satisfaction
