About the Role:
As a Home Care Specialist, you will be responsible for delivering exceptional care and support to our clients, including:
• Providing personal care services, such as bathing, grooming, and dressing
• Assisting with daily activities, such as meal preparation, light housekeeping, and medication management
• Offering companionship and emotional support to enhance our clients' quality of life
• Collaborating with our care team to develop and implement personalized care plans
• Maintaining accurate records of client care and services provided
Responsibilities:
• Conduct initial client assessments to identify care needs and develop personalized care plans
• Provide care and support services in accordance with client care plans and agency policies
• Maintain a clean, safe, and organized work environment
• Communicate effectively with clients, families, and care team members
• Participate in ongoing education and training to stay current with industry best practices and regulations
