Responsibilities:
Assess the emotional, social, and psychological needs of residents and their families
Develop and implement personalized care plans to address identified needs
Provide emotional support and guidance to residents and families
Facilitate communication among healthcare team members
Participate in interdisciplinary care planning and coordination
Document resident assessments, care plans, and progress
Collaborate with other healthcare professionals to ensure resident-centered care
Requirements:
Bachelor's degree in Social Work (BSW) or related field
