Client: 89-year-old male

Condition: Fractured pelvis (post-rehab discharge)

Living Situation: Lives with spouse and one small dog
-
Start of Care: Saturday (discharge day)
Care Needs
- Hands-on assistance with mobility
- Bathing and personal care
- Housekeeping and laundry support
-
Transportation support (may require caregiver to drive client’s vehicle for errands/appointments)
- Companionship and in-home support
Requirements:
- '14;️ Meet & Greet required before starting
- '14;️ Pass a background check
- '14;️ Pass a drug screening
- '14;️ Reliable transportation and commitment to the assigned schedule
