Responsibilities:
Provide personalized care and support to clients in their homes, including assistance with daily living activities, medication management, and companionship
Develop and maintain strong relationships with clients, families, and caregivers to ensure exceptional care and communication
Collaborate with clients, families, and healthcare professionals to create personalized care plans
Perform light housekeeping, meal preparation, and other household tasks as needed
Maintain accurate and timely records of client care and services
Participate in ongoing training and professional development to stay up-to-date on best practices and industry developments
