Pulmonary Program Coordinator (RN)
Salary: $87,360- $91,520 annually (depending on experience)
Hourly Equivalent: $42- $44
Shift: Full-Time

Taconic Rehabilitation & Nursing at Ulster is seeking an experienced Pulmonary Program Coordinator (RN) to oversee and coordinate our Pulmonary Program. In this role, you'll provide clinical leadership, coordinate pulmonary-focused care, and collaborate with the interdisciplinary team to support residents with respiratory and pulmonary needs.

RESPONSIBILITIES:
  • Oversee and coordinate daily operations of the Pulmonary Program
  • Coordinate individualized care plans for residents with pulmonary and respiratory needs
  • Monitor residents with respiratory and pulmonary conditions
  • Collaborate with physicians, respiratory providers, therapy, nursing, and interdisciplinary teams
  • Assist with admissions, assessments, and care coordination for pulmonary residents
  • Provide staff education on pulmonary care and clinical best practices
  • Ensure compliance with regulatory and clinical standards
  • Participate in quality improvement and program development initiatives
REQUIREMENTS:
  • Active New York State Registered Nurse (RN) license required
  • Skilled nursing, rehabilitation, pulmonary, respiratory, or subacute experience preferred
  • Strong clinical assessment and care coordination skills
  • Experience with care planning and interdisciplinary collaboration preferred
  • Strong leadership, organizational, and communication skills
