As we launch our new hospice program in Billings, we are seeking a compassionate and dedicated Hospice Social Worker to help support patients and families during one of life's most meaningful journeys.

Position Summary

Everwind Hospice is seeking a PRN Hospice Social Worker (LCSW/LMSW) to provide compassionate psychosocial support to hospice patients and their loved ones. Working as part of an interdisciplinary team, you will assess patient and family needs, provide counseling and education, connect families with community resources, and advocate for patient-centered care. This flexible PRN position is ideal for someone who is passionate about making a lasting impact while enjoying schedule flexibility.

Responsibilities
  • Complete psychosocial assessments and develop individualized plans of care.

  • Provide emotional, psychological, and supportive counseling to patients, families, and caregivers.

  • Assist patients and families with advance care planning, community resources, financial concerns, and end-of-life decision-making.

  • Identify and address social determinants of health that may impact patient care and quality of life.

  • Provide crisis intervention and emotional support during periods of change or loss.

  • Collaborate with physicians, nurses, chaplains, hospice aides, volunteers, and other members of the interdisciplinary team.

  • Educate patients and families regarding hospice services, disease progression, coping strategies, and bereavement resources.

  • Maintain timely, accurate documentation in the electronic medical record in accordance with agency policies and regulatory requirements.

  • Participate in interdisciplinary team meetings as requested.

Qualifications
  • Master's Degree in Social Work (MSW) from an accredited program required.

  • Current Montana Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) eligible to practice independently within the hospice setting, as permitted by Montana law.

  • Hospice, palliative care, home health, healthcare, or medical social work experience preferred.

  • Excellent communication, counseling, and interpersonal skills.

  • Ability to work independently while collaborating effectively within an interdisciplinary team.

  • Strong organizational, documentation, and critical thinking skills.

  • Valid driver's license, reliable transportation, and current auto insurance.
