Job Summary:
We are seeking a compassionate and experienced Social Worker to join our team at Mountain View Rehabilitation and Senior Living Center. As a Social Worker, you will work closely with our interdisciplinary team to provide holistic care to our residents, including those with physical and cognitive impairments. Your responsibilities will include:
Conducting social histories and assessments to identify resident needs and develop individualized care plans
Providing counseling and therapy services to residents, families, and caregivers
Coordinating care with other healthcare professionals and community resources
Facilitating group activities and events that promote socialization and engagement
Assisting with discharge planning and transition services
Maintaining accurate records and documentation
Requirements:
Bachelor's degree in Social Work
Possesses a strong understanding of social work theory and practice
Ability to work effectively in a fast-paced environment
Excellent communication and interpersonal skills
Strong problem-solving and crisis management skills
Ability to maintain confidentiality and professionalism at all times
