The Pavilion at Kenton, a warm and welcoming skilled nursing facility in historic Covington, KY, is seeking an experienced MDS Coordinator to join our team. Our close-knit community — complete with a majestic historic chapel, courtyard healing garden, and café — is dedicated to enriching residents' lives in a gracious, home-like environment. The MDS Coordinator oversees the Minimum Data Set (MDS) assessment process and plays a key role in resident care planning and reimbursement accuracy. Prior nursing home experience is required for the MDS Coordinator position.

MDS Coordinator Duties
Complete and coordinate MDS assessments accurately and on schedule
Manage the RAI process and ensure compliance with federal and state requirements
Collaborate with the interdisciplinary team on resident care plans
Monitor documentation to support accurate PDPM reimbursement
Track assessment schedules and submission deadlines

MDS Coordinator Qualifications
Registered Nurse (RN) or Licensed Practical Nurse (LPN) license
Prior MDS experience required
Must have experience in a skilled nursing facility (SNF) / long-term care (LTC) setting
