OUD REENTRY SPECIALIST POSITION SUMMARY:


The OUD (Opioid Use Disorder)-Mental Health Reentry Specialist is responsible for providing case management, reentry planning, and transitional support services to incarcerated individuals with substance use disorders, mental health needs, and co-occurring disorders. The specialist works collaboratively with correctional staff, treatment providers, community agencies, and other stakeholders to ensure individuals have access to services, resources, and supports that promote successful reintegration into the community and reduce recidivism.

OUD REENTRY SPECIALIST RESPONSIBILITIES:

  • Conduct comprehensive reentry assessments to identify behavioral health, substance use, housing, employment, educational, medical, and social service needs.
  • Develop individualized reentry plans that address treatment, recovery, and community stabilization goals.
  • Provide case management and ongoing support to incarcerated individuals preparing for release.
  • Coordinate pre-release services, including referrals to community-based treatment providers, medication-assisted treatment (MAT) programs, mental health services, housing resources, and employment programs.
  • Facilitate continuity of care between correctional facilities and community providers.
  • Assist individuals in obtaining identification documents, benefits, insurance coverage, and other essential resources needed for successful reentry.
  • Collaborate with correctional healthcare staff, probation and parole officers, courts, treatment providers, and community organizations to support successful transitions.
  • Provide education regarding substance use disorders, recovery supports, relapse prevention, and mental health wellness.
  • Monitor participant progress and document all case management activities in accordance with agency policies and regulatory requirements.
  • Maintain accurate records, reports, and data related to program outcomes and participant engagement.
  • Advocate for participants while promoting accountability and compliance with supervision and treatment requirements.
  • Participate in multidisciplinary team meetings and case reviews.
  • Assist with crisis intervention and support individuals experiencing behavioral health challenges.
  • Maintain confidentiality and adhere to all applicable federal, state, and agency regulations.

OUD REENTRY SPECIALIST MINIMUM REQUIREMENTS:

  • Bachelor's degree in Social Work, Psychology, Criminal Justice, Human Services, Counseling, Behavioral Health, or a related field required.
  • Minimum of two years of experience working with justice-involved individuals, substance use treatment, mental health services, case management, or reentry services.
  • Knowledge of Opioid Use Disorder, Medication-Assisted Treatment (MAT), mental health disorders, trauma-informed care, and recovery-oriented practices.
  • Experience coordinating services with community agencies and healthcare providers.
  • Strong interpersonal, communication, and organizational skills.
  • Ability to establish rapport with diverse populations and maintain professional boundaries.
