REQUIREMENT: NURSING HOME EXPERIENCE
Come join our team! Social Work/Case Manager for a beautiful skilled nursing facility. Knowledge of Medicare and Medicaid, MLTC, HMO Case Management, Discharge Planning are required. Prior Nursing Home Experience strongly preferred.
