Job Description

Job Description
Regional Patient Care LiaisonHealthcare Sales | Hospital Partnerships | Skilled Nursing & Post-Acute Care

MA | In-Market Leadership Role

You've Proven You Can Grow a Market. Are You Ready to Lead One?

We are looking for an experienced Regional Patient Care Liaison who brings the energy of a top-performing healthcare sales professional and the leadership skills to help others succeed.

This is a hands-on, field-based leadership opportunity for someone who knows skilled nursing and post-acute care, understands hospital discharge planning, and knows how to turn strong relationships into referrals, admissions and census growth.

If you're currently a successful Patient Care Liaison, Hospital Liaison, Clinical Liaison, Healthcare Marketer, Admissions Liaison or Post-Acute Sales professional looking for your next opportunity to grow, this may be it.

You'll lead from the field—not from behind a desk.

What You'll Get to Do
  • Lead market growth across multiple MA Skilled Nursing Facilities

  • Develop relationships with hospitals, case managers, discharge planners, physicians, social workers and payors

  • Work alongside Patient Care Liaisons to strengthen account strategy and referral development

  • Coach and mentor team members on sales strategy, conversion, follow-up and relationship development

  • Identify new referral opportunities and expand existing healthcare partnerships

  • Help solve barriers that stand between a qualified referral and an admission

  • Use referral and market data to identify opportunities and create targeted growth strategies

  • Partner with facility and regional leadership on census, skilled and Medicare growth

  • Maintain your own presence in the market and lead by example

We're Looking for a Relationship Builder AND a Results Driver

You know that great healthcare business development is more than making visits.

It's knowing your market. Building trust. Understanding what your referral partners need. Following up when others don't. Being responsive when a discharge planner needs an answer. Knowing your facilities' clinical capabilities—and confidently communicating why your team is the right choice for a patient.

And then helping your team do the same.

You Could Be a Great Fit If You Have
  • 2+ years of experience in Skilled Nursing, Post-Acute Care, Healthcare Sales, Hospital Liaison, Clinical Liaison, Admissions, Healthcare Marketing or Business Development

  • Proven success generating referrals, admissions and census growth

  • Experience developing hospital and healthcare referral relationships

  • Knowledge of Medicare, Managed Care, Skilled Nursing and the hospital discharge process

  • Experience coaching, mentoring or supporting other sales or liaison professionals

  • Strong territory management and account development skills

  • Excellent communication and relationship-building skills

  • A sense of urgency, strong follow-through and a solutions-focused mindset

  • The ability to work independently while collaborating with facility and regional teams

Experience in SNF, Skilled Nursing, Home Health, Hospice, LTACH, Inpatient Rehabilitation, Assisted Living, Senior Care or other Post-Acute Healthcare environments is a plus.

What Success Looks Like

Success isn't measured by how many places you visit.

It's measured by the relationships you build, the opportunities you create, the referrals your team earns and the patients you're able to connect with the right level of care.

You'll help build:

Stronger Hospital Partnerships → More Qualified Referrals → Better Conversion → More Admissions → Census Growth

Ready for What's Next?

Maybe you've been the liaison everyone calls for help.

Maybe you're the person who knows how to open a difficult account, turn around a referral relationship or get a challenging admission across the finish line.

Now you're ready to expand your impact.

Bring your relationships. Bring your healthcare experience. Bring your drive to grow.

Sweetwater Care will give you the opportunity to lead from the front and help build what's next.
