Community Based Service Provider - Bilingual

Are you passionate about making a real difference in your community? As a Community-Based Service Provider (CBSP), you will deliver direct services to children and their caregivers enrolled in Children & Family Treatment Supports and Services (CFTSS) and Home & Community Based Services (HCBS) programs. Using a person-centered planning approach, you will engage youth and caregivers to develop personalized treatment plans and identify future goals.Responsibilities:Provide direct, goal-focused services to assigned youth and caregivers. Develop and implement person-centered treatment plans based on the strengths and needs of the child and family. Assist youth and families in identifying and achieving treatment goals. Complete treatment plans, safety plans, progress notes, and all required documentation accurately and within established timelines. Submit service-related notes within 48 to 72 hours of service delivery. Coordinate services and collaborate with schools, healthcare providers, community agencies, and other supports involved in the family's care. Participate in interdisciplinary team meetings and case consultations. Maintain a service calendar documenting all scheduled, completed, missed, or rescheduled services. Communicate effectively with Managed Care Organizations and other stakeholders as required.
