Responsibilities:

  • Coordinate and complete the Minimum Data Set (MDS) process for all residents, ensuring timely and accurate completion of assessments, care plans, and updates

  • Collaborate with interdisciplinary teams, including physicians, therapists, and other healthcare professionals to develop and implement individualized care plans

  • Collect and analyze data, identifying trends and areas for improvement, and presenting findings to the care team

  • Participate in quality improvement initiatives, supporting the development and implementation of programs to enhance resident outcomes and satisfaction

  • Maintain accurate and detailed records, ensuring compliance with regulatory requirements and internal policies

  • Provide education and support to residents, families, and staff on MDS processes, care plans, and resident needs

Requirements:

  • Currently licensed as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Illinois
