About the Role:
We are seeking a dedicated and compassionate Weekend On-Call Registered Nurse to join our team in Chicopee, Massachusetts. As a member of our team, you will provide exceptional patient care and support to our patients and their families on weekends and during on-call rotations. Responsibilities:
Provide skilled nursing care to patients in their homes, hospitals, and hospice facilities
Develop and implement individualized care plans in collaboration with the interdisciplinary team

Communicate effectively with patients, families, and healthcare providers to ensure timely and effective treatment
Participate in on-call rotations, providing 24/7 support to patients and families
Collaborate with the healthcare team to ensure seamless transitions and quality care
