Your RoleAs a Home Health Registered Nurse, you’ll provide skilled nursing care to patients in their homes while coordinating with physicians, families, and other members of the interdisciplinary team to help patients achieve the best possible outcomes.
Key ResponsibilitiesConduct comprehensive in-home patient assessments
Develop and update individualized plans of care
Administer medications, treatments, injections, and wound care according to physician orders
Monitor patient progress and identify changes in condition
Communicate changes and concerns with physicians and the interdisciplinary care team
Educate patients and families on medications, disease management, safety, and wellness strategies
Coordinate care with physical therapists, occupational therapists, speech therapists, social workers, and other healthcare professionals
Supervise and document care provided by LPNs and Home Health Aides (HHAs), as appropriate
Maintain timely, accurate, and compliant clinical documentation
Help patients safely maintain their independence and quality of life at home
Current Arizona RN license in good standing
Minimum of one year of RN experience preferred
Home health, hospice, rehabilitation, med-surg, or critical care experience is a plus
Current CPR certification
Valid driver’s license and reliable transportation
Strong clinical assessment and critical-thinking skills
Excellent communication, organizational, and interpersonal skills
Ability to work independently while collaborating with an interdisciplinary team
