Job Summary:

As a Registered Nurse (RN) Case Manager - Hospice, you will have the opportunity to make a meaningful difference in the lives of our patients and their loved ones.
Key Responsibilities:
Perform initial and ongoing assessments of patients and their families to develop individualized plans of care
Coordinates care with the interdisciplinary team, including physicians, social workers, and other healthcare professionals
Provides ongoing patient and family education and support to ensure optimal care and quality of life
Identifies and addresses potential barriers to care, such as psychosocial and spiritual concerns, and develops strategies to overcome them
Collaborates with the hospice team to ensure seamless care transitions and effective communication with all involved parties
Maintains accurate and timely documentation of patient care and communication with the healthcare team
Requirements:
Current licensure as a Registered Nurse (RN) in the state of Pennsylvania
Strong clinical knowledge and skills, with a focus on hospice and palliative care
Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
Strong organizational and prioritization skills, with the ability to manage multiple responsibilities and deadlines
Ability to work in a fast-paced environment and adapt to changing situations and priorities
