Responsibilities:
Conduct accurate and thorough MDS assessments of residents, identifying their physical, emotional, and social needs
Develop and implement individualized care plans that address the unique needs of each resident
Collaborate with other healthcare professionals to deliver high-quality care and achieve optimal outcomes
Communicate effectively with residents, families, and colleagues to ensure seamless transitions and continuity of care
Participate in quality improvement initiatives and contribute to the development of policies and procedures

Experience with MDS in a Long Term Care Environment
