Job Description
A well-established skilled nursing facility in Far Rockaway, Queens is hiring an experienced MDS Coordinator. This is a full-time MDS Coordinator role paying up to $150,000 per year, and it requires a Registered Nurse (RN) with prior MDS Coordinator experience in a skilled nursing setting — this is not a training position.

Our MDS Coordinator plays a central role in resident care planning and facility reimbursement, working closely with nursing leadership and the interdisciplinary team.
What You'll Do
Complete and coordinate MDS (Minimum Data Set) assessments accurately and on schedule
Manage the RAI (Resident Assessment Instrument) process and lead care plan meetings with the interdisciplinary team
Ensure documentation supports accurate PDPM (Patient-Driven Payment Model) reimbursement
Maintain compliance with federal and New York State assessment requirements
Work with nursing and therapy teams to keep assessments aligned with residents' actual clinical status
Qualifications
Current Registered Nurse (RN) license in New York State — required
Prior experience as an MDS (Minimum Data Set) Coordinator — required
Experience in a skilled nursing facility (SNF) / long-term care (LTC) setting — required
Compensation
Up to $150,000 per year, based on experience
