Nurse Practitioner NP

Avir at Rose Trail and Avir at Petal Hill are seeking a Nurse Practitioner to provide on-site clinical care between both facilities in Tyler, Texas.

This is a facility-based role focused on managing post-acute and long-term care residents within a skilled nursing environment. The Nurse Practitioner will work closely with physicians and nursing leadership to support strong clinical outcomes and reduce hospitalizations.

Key Responsibilities
• Provide comprehensive medical care to residents
• Perform assessments, follow-ups, and manage acute changes in condition
• Collaborate with physicians and interdisciplinary teams
• Participate in care planning and clinical meetings
• Ensure timely and accurate documentation
• Support quality initiatives and rehospitalization reduction efforts

Qualifications
• Active Texas Nurse Practitioner license
• Nursing home or long-term care experience required
• Strong clinical assessment and decision-making skills
• Ability to work independently within a structured clinical model
